Immunizations

  • K.A.R 28-1-20 defines immunizations required for any individual who attends school or a childcare program operated by a school.  

  • Proof of receiving the immunizations must be provided to the school prior to attending the first day of school.

Kansas Certificate of Immunizations Form

 Legal alternatives to immunizations include:

  • Kansas Certificate of Immunizations Medical Exemption Form B completed annually by a licensed physician.

  • Religious Exemption letter signed by a parent/guardian stating that the student is adherent to religious beliefs opposing vaccination. Kansas Law does not permit exemptions to vaccination solely on the grounds of personal or philosophical beliefs.

School Physical Requirement

Students less than 9 years of age entering a Kansas school for the first time must present a health assessment (physical) in accordance with Kansas Statute 72-5214.
